Use this form to respond to a request for your neighborhood Annual Operating Fund (sometimes called Landscape Fund) from an OPAL-associated treasurer or other authorized individual. Complete the online form or submit the PDF form to the OPAL Office Manager via mail, email, or in-person at the OPAL office.
